Scenario 8 A 48 year old female is admitted with necrotizing fasciitis, debrided in the operating room and started on penicillin and clindomycin. 24 hours after admission she develops an extensive skin rash, and simultaneously becomes gradually anuric. Her creatinine rises to 3.0 over the course of 48 hours. Her renal injury is unresponsive to fluid loading, vasopressors and diuretics. What is your diagnosis, and how would you treat this patient? |
