Vasomotor nephropathy is a generic term that is applied to conditions in which renal dysfunction occurs because of an impairment or imbalance in renal vasomotor control, rather than an intrinsic renal injury, such as acute tubular necrosis--although this may certainly be a late consequence. In certain of these situations, notably acute tubular necrosis itself, severe sepsis, and possibly during cardiopulmonary bypass, autoregulation seems to be lost or attenuated. Renal blood flow is reduced strikingly during hypotension, and is restored by normalization of renal perfusion pressure, even if this is achieved by vasoconstrictor therapy.
