Do incremental costs and effects differ between subgroups?

The answer to this question is driven primarily driven by the proportion of patients potentially able to benefit from therapy. Many clinical trials are constructed from highly selected patient populations in order to create uniformity (and statistically significant results), in a specific practice environment (a university hospital usually). This may make the application of the results to subgroups, other than those studied, difficult.
