Clinical Scenario A 52 year old male is admitted with abdominal pain, hypotension (BP 70/30, pulse 150), hypoxemia (PaO2 50mmHg on 60% oxygen), acidosis (pH 7.21, base deficit -10), oliguria, pyrexia and leucocytosis. On chest x-ray there are diffuse infiltrates throughout the lung fields and free air under the diaphragm. What do you think? |
